We are pleased to provide the FamilyCare Medicaid Formulary as a useful reference and informational tool. The FamilyCare Medicaid Formulary can assist practitioners in selecting clinically appropriate and cost-effective drugs and products for their patients.

The drugs represented have been reviewed and approved by FamilyCare’s Pharmacy and Therapeutics (P&T) Committee. The FamilyCare Medicaid Formulary is reflective of current medical practice and the Oregon Health Authority’s coverage guidelines as of the date of review.

All the information in the FamilyCare Medicaid Formulary is provided as a reference for drug therapy selection. Specific drug selection for an individual patient rests solely with the prescriber.

National guidelines can be found on the National Guideline Clearinghouse site at http://www.guideline.gov or on the websites listed under each therapeutic class.
