Becoming a member of FamilyCare Health Plans is easy! If you need help at any point, just call one of our friendly customer service representatives, who will guide you through every step of the way.

Feel free to:

1) Enroll online via our secure application process after October 15, 2011.

2) Complete the Application Form and mail or fax it to us at using the information listed below.

3) Enroll by phone. Call us  Monday through Friday from 8 am to 8 pm at:

Once FamilyCare has received your enrollment form:

  • We will verify your enrollment.
  • We will call you and let you know you've been accepted.
  • We will send you a Confirmation of Enrollment Letter and a New Member Packet.

Caseworker Tools
The Oregon Health Authority's Medicare Advantage Plan Election Form (7208M) is available by visiting Oregon.gov and selecting OHP 7208M. The form can be completed electronically then printed, signed and mailed to the DMAP medical plan.
