*Deductibles and co-pays vary based on the level of Extra Help you receive. Premiums, co-payments and co-insurance, out-of-pocket limits, benefits and other details may change on January 1, 2013. For complete details, see the Evidence of Coverage. You must continue to pay your Part B premium. The benefit information provided herein is a brief summary, not a comprehensive description of benefits. For more information contact the plan.
PremierCare Plus has formed a network of doctors, specialists and hospitals. You can use any of the providers, hospitals and pharmacies who participate in the FamilyCare network except in emergency or urgent care situations or for out-of-area renal dialysis or other services. If you obtain routine care from out-of-network providers neither Medicare nor FamilyCare will be responsible for the cost. Members must use network pharmacies to access their prescription drug benefit, except under non-routine circumstances, and quantity limitations and restrictons may apply.
FamilyCare Health Plans is a coordinated care plan with a Medicare Advantage contract and a contract with the Oregon Medicaid program.
